There is a ramp to the front door for wheelchair users. We are currently having a lift installed enabling full access across the surgery for all patients.

Confidentiality
The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances:
- To provide further medical treatment for you, eg from district nurses and hospital services.
- To help you get other services, eg from the social work department. This requires your consent.
- When we have a duty to others, eg in child protection cases.
Anonymised patient information will also be used at local and national level to help the CCG and Government plan services, eg for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know.
Connecting Your Care
What do we mean by “Connecting Your Care”?
We are working with South West London Health and Care Partnership to improve the way we connect your care across south west London. Soon, healthcare professionals will be able to access your records from other NHS services when you need them to. This will make it quicker and easier for you when you visit your GP or hospital.
At the moment, each care organisation has a different system for managing your records, and there is no way for the information to be shared electronically when it is needed by doctors and nurses.
Connecting your Care means that people involved in your care such as your GP, hospital doctors and nurses, will be able to immediately see important information about you through a secure system, to help them make the best decisions about your care.
In the future, we will also be working with other health and social care providers to share a more detailed care record for you, which will include the treatment you receive from community NHS services, mental health services and social care services. This will allow everyone involved in your care to be able to make the best decisions for your treatment.
Do I have a choice?
If you are happy for your information to be shared in this way, then you don’t need to do anything. The Connecting your Care system will be phased in over the next few months.
If you do not want your information to be shared, you have the right to opt-out by visiting www.swlondon.nhs.uk/connectingyourcare and downloading the opt-out form.
You can also pick up an opt-out form from your GP practice or the Patient Advice and Liaison Services (PALS) at your local hospital.

General Data Protection Regulation
General Data Protection Regulation (GDPR) laws have changed recently bringing about different ways of working within General Practice. This will change the way we operate making it easier for you to obtain details on how your information is handled giving more rights to you the patient as to how you can access your information.
As a result, we’ve published a new privacy notices to make it easier for you to find out how the East Croydon Medical Centre uses and protects your personal information.
The EU General Data Protection Regulation (GDPR) requires that data controllers provide certain information to people whose information (personal data) they hold and use. A privacy notice is one way of providing this information. This is sometimes referred to as a fair processing notice.

Net GP Earnings
All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.
The average pay for GPs working in Edridge Road Practice in the last financial year was £29,428 before tax and National Insurance.

The Rights and Responsibilities of Patients
Patients’ Rights
You have the right to:-
- Receive health care on the basis of clinical need, regardless of your income.
- Equal treatment, regardless of race, gender, age or disability or nature of your health problems as long as you reside within the practice boundaries and qualify for NHS treatment (details of the practice boundary can be obtained from the reception staff).
- Have reasonable access to high quality service and facilities.
- Receive information on what is wrong with you and the treatment options available.
- Ask questions and be given truthful and clear answers.
- Ask for a second opinion on your diagnosis or treatment.
- Ask to have someone with you (friend, relative or interpreter) at any time. You may find this beneficial, particularly if you are asking questions or need moral support.
- Register with a Medical Practice. If a Practice is unwilling to take you onto their list you should be given a written reason for this decision. NHS Croydon can arrange for you to be registered with a Practice, please phone 020 8335 1400 for this service. You are entitled to change your Practice at any time. You do not have to give a reason for your decision.
- Refuse treatment as you see fit. This includes examinations, tests, diagnostic procedures, medications, operations etc.
- Refuse to be examined or treated in the presence of medical students.
- Refuse to be involved in research trials.
- Information on GP Practices in your area and the services they provide. All practices must provide an information leaflet.
- Confidentiality – personal information about your health is confidential and should only be disclosed to those who need that information to provide you with effective treatment.
- Access all records stored on computer. You are entitled to copies of your records; however, a charge may be levied if deemed excessive. If you wish to see your records you should apply in writing to the General Manager.
Human Rights
It is your right to be treated as a human being by another human being as they would wish to be treated themselves, ie with dignity, politeness, respect and consideration.

Patients’ Responsibilities
Patients have certain responsibilities:-
- Keeping appointments. Please try to arrive on time. If you are unable to attend an appointment, please inform the clinic or surgery in good time. Our 24/7 telephone and web booking systems are available for such eventuality.
- Treat all healthcare staff in a reasonable, courteous manner.
- Use emergency services in a responsible manner. Please use the out-of-hours services for emergencies only and not for routine care.
- Take care with medicines. Medicines are for the intended person only and should not be shared. Keep them safely away from children and in the original container. Take any unwanted medicines to a chemist for safe disposal.
- Inform the practice of any change of name, address, telephone number or email.
- Keep young children in your care under constant supervision and ensuring that they behave appropriately at all times whilst on the surgery premises.
- Switch off mobile phones whilst on the surgery premises.
- As patients, you are responsible for your own health and that of any dependants. It is important that you adhere to information and advice given to you by health professionals, and co-operate with the practice in endeavouring to keep you healthy.
Freedom of Information
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
Violence/Abuse
The practice considers aggressive behaviour to be any personal, abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures.
The practice will request the removal of any patient from the practice list who is aggressive or abusive towards a doctor, member of staff, other patient or who damages property.
All instances of actual physical abuse on any doctor or member of staff, by a patient or their relatives will be reported to the police as an assault.
Practice Safeguarding
Safeguarding Adults
Safeguarding means protecting an adult’s right to live in safety, free from abuse and neglect.
It is about people and organisations working together to prevent and stop both risks and experience of abuse or neglect, whilst at the same time ensuring the adult’s wellbeing is promoted.
Also, where appropriate, having regard to their views, wishes, feelings and beliefs in deciding on any action.
This must recognise that adults sometimes have complex interpersonal relationships and may be uncertain and have unclear or unrealistic views about their personal circumstances.
If you have any concerns about a vulnerable adult, please speak to your GP or Practice Nurse.
Safeguarding Children
The East Croydon Medical Centre is committed to protecting Children and Vulnerable Adults and we will make their welfare our highest priority. Where additional support is necessary this will include working with other agencies.

If you have any suggestions on how we can improve the way you are looked after, please put these in writing addressed to the general manager.
Complaints
We make every effort to give the best service possible to everyone who attends our practice. However, we’re aware that things can go wrong, resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. Simply contact the General Manager who will set all the necessary wheels in motion.

ECMC and Edridge Road Complaints Form
Further written information is available on the complaints procedure from reception. However, if you feel we have not dealt with the issues you have raised as you would wish, you can contact the Patient Advice and Liaison Service (PALS), which can often help resolve any problems before they become formal complaints. To speak to a PALS officer, ring 020 8274 6333.
The National Data Pull (GPDPR) or Data Sharing Opt-Out
NHS Digital has issued a DATA Provision Notice for planning and research.
This means we are required to share your information with them unless you tell us not to.
